on-call Description: Registered Nurse needed for night shift from 7
PM to 7 AM. This role involves the overall management of nursing
care provided, assuring competent and compassionate patient care
from admission to discharge. The nurse is responsible for patient
safety, comfort, personal hygiene, and protection, while also
performing disease prevention and restorative measures. The nurse
must respond to medical and environmental emergencies. The nurse
will provide competent, compassionate, and cost-effective patient
care in accordance with policies and standards of care. Planning
and coordinating delivery of care to ensure patient safety and
comfort is essential. Setting priorities and providing care in
routine and complex situations is required. The nurse serves as a
liaison between nursing and medical staff to facilitate
collaborative patient care. Involvement of patients, physicians,
families, and health team members must be coordinated effectively.
Setting priorities and initiating emergency interventions in a calm
and competent manner are crucial. The registered nurse must
exercise clinical judgment and critical thinking across all aspects
of the nursing process. Timely administration of treatments,
medications, and nursing skills is expected. Maintaining current
knowledge of patient care status and monitoring patient perceptions
of care through clinical rounds is important. The nurse must inform
physicians of any significant changes in patient conditions.
Analyzing and documenting patient acuity levels accurately and in a
timely manner is a key responsibility. Initial and ongoing patient
assessments must be conducted, considering various factors
including psychosocial and self-care needs. The nursing assessment
must be completed within 24 hours of admission. The nursing process
should be individualized to meet all patient needs, including
education and discharge planning. Monitoring patients at risk for
adverse events is essential, and precautions should be implemented
accordingly. Evaluating the effectiveness of the care plan through
observation and communication is important. Modifications to the
care plan must be made in collaboration with the patient, family,
and other disciplines. The development of the patient's plan of
care requires collaboration with the patient, family, physicians,
and other team members. Documenting and communicating patient
assessments, plans of care, outcomes, and pertinent information is
critical. Outcomes of the patient's care plan must be evaluated and
followed up on, with modifications communicated to relevant parties
