Description Clinical Pharmacist at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is full time and will work 10-hour, Night shifts. The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Required Qualifications: - PharmD in Pharmacy Doctorate Or - Bachelor's Degree in Pharmacy - Upon hire: California Pharmacist Registered - Upon hire: Basic Life Support (BLS) (Facility Managed) - where applicable Preferred Qualifications: - Acute care hospital pharmacy experience, 1 year or equivalent experience. - Completion of ASHP-accredited PGY1 residency. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
